REPORT OF MARRIAGE



For registration of marriage with the National Statistics Office  (NSO) and to allow married name in Passport

To lodge a Report of Marriage to the Philippines Embassy the requirements and application form can be downloaded and viewed by clicking the link directly below:


1.   Duly-accomplished Report of Marriage application form.  (Click Here to download the  application form)

2.   Certified true copies of the following documents:

     Marriage record/ marriage certificate issued by the Civil Registrar or Registry of Births, Deaths and Marriages of the state/ territory where the 
     marriage took place.


     Birth record/ birth certificate of the Filipino spouse.

     Passport's data page of the Filipino spouse.

3.   For women who obtained annulment or were divorced by the foreign husband, certified true copy of the annulment or divorce certificate, and a
     Report of Divorce. (see separate requirements)

4.   For widows/ widowers, death certificate of the deceased spouse. (If spouse died in the Philippines, the certified true copy of the NSO-   
     authenticated copy must be submitted.)

5.   Passport-size photos of the husband and wife (1each).

6.   Processing fee of AUD45 (in the form of a postal money order or bank cheque made payable to the Philippine Embassy.

7.   A self-addressed, pre-paid return envelope.


(Note: Other documents such as proof of Philippine citizenship might be required when deemed necessary.)
