Venue is Rear of 112 Mounts Bay Road, Perth
The Passport processing will be held 10am to 4pm 9th to 10th December, no appointment required, first come first served. You MUST bring with you the following:
1. Completed application form - click here for PDF format or here for WORD format (leave form unsigned)
2, Copy of Bio-Data page of current Passport,
3. A copy of current Australian Visa page in your Passport,
4. Payment in Cash is accepted (must be exact amount as change may not be available) or else by Postal Order
payable - to Philippines Embassy for $118.00 , (purchase this from any Post Office)
You can attend for your personal appearance in Perth at the address ( Rear of ) 112 Mounts Bay Road, Perth, anytime between 10am and 4pm on the 9th and 10th December, 2011.
Note if you have since married in Australia and want the new Passport in your married name you must first report your Marriage to the National Statistics Office and this can be done by follow the instructions by clicking here:
But for those who have been in Australia for 4 (four) years or more and presently have an expired passports and are now a Permanent Resident of Australia they require a Certificate of Residency from Australian Department of Immigration and Citizenship to accompany the application .This can be applied for with the form as per the link below

Nearest pubic parking is at the Perth Convention Centre which is also on Mounts Bay Road and only a few minutes walk from the Consulate.
